Code cleanup.

* mandos (ForkingMixInWithPipe.process_request): Call "self.add_pipe".
  (ForkingMixInWithPipe.handle_ipc): Removed.
  (ForkingMixInWithPipe.add_pipe): New.
  (MandosServer.add_pipe): New.
